Severe Discrepancies and Lingual Orthodontics: Camouflage vs Orthognathic Surgery

Severe discrepancies represent the most challenging malocclusions for every orthodontic clinician. The presence of sagittal, vertical and transverse components, with a variable amount of skeletal asymmetry, make in many cases the combination of orthodontics and maxillofacial surgery mandatory. In other cases, it is possible to obtain a favorable result with an orthodontic camouflage, with inevitable dental compensations: the functional result is sometimes combined with a significant esthetic improvement. What is the limit for a camouflage treatment? The criteria to establish if a patient can be successfully treated with an orthodontic camouflage or needs orthognathic surgery will be presented, highlighting the advantages and disadvantages of the two different approaches. This lecture aims in addition to clinically demonstrate that lingual orthodontics can enable the resolution of complex malocclusions with severe discrepancies. Through a short sequence of clinical cases, belonging to different severe malocclusions, the clinical effectiveness and biomechanical advantages of lingual technique will be evidenced.
